blood_pressure3If you have a chronic disease such as Asthma, COPD, Heart Disease, Diabetes or have had a stroke in the past, you will be invited for an annual health check.

You will be given a 10 minute appointment with a Healthcare Assistant and, depending on your illness, you may require a follow-up 20 minute appointment one week later with the Clinical Pharmacist.  You will be advised by letter when your check is due, but if you think you may have been missed, please check with Reception. We will let you know if you need a blood or urine test or if you need to bring anything with you.

Annual hypertension checks can be carried out by the Healthcare Assistants.

If you are happy that your asthma is properly controlled by your current medication, you can let us know by filling out the form found in the link on the right.

If you have any concerns about any aspect of your chronic disease, please make an appoinment to see your GP or the Practice nurse.

You do not have to wait until you are called.
